List of Countries/Regions that Accept Vaccine Passports
Eligibility
Persons whose residence was registered in Kanazawa City at the time of COVID-19 vaccination.
Application Method
Please apply via post.
Required Documents

| Passport photocopy | The page showing your identification information. |
| Photocopy showing proof of completed vaccination, or photocopy of vaccination record | The “vaccination record” is for medical staff and some workers at aged care facilities for the elderly. |
| Document showing proof of residence | Photocopy of driver’s license, health care card, etc. Passports are acceptable if your residence certificate shows your address. |
| Reply envelope with postage stamp | Please write your name and address on an empty envelope, and affix a postage stamp. |
